Am I a Candidate for Medical Weight Loss?

Medical weight loss may be appropriate if you have an elevated BMI, weight-related health conditions, or prior attempts that have not produced lasting results.

Who May Benefit from Medical Weight Loss?

One of the most common questions I hear from new patients is straightforward: "Am I actually a candidate for this, or should I keep trying on my own?" It is a fair question. Medical weight loss is a clinical service, not a universal solution, and candidacy depends on your health profile, goals, and whether supervised treatment offers meaningful advantages over self-directed efforts.

General guidelines used in obesity medicine suggest that medical weight loss may be appropriate for adults who meet certain criteria. These guidelines serve as a starting point for conversation—not a rigid gatekeeping system. Your provider evaluates the full context of your health during a consultation.

Common Candidacy Considerations

Medical weight loss is frequently considered for patients who fall into one or more of the following categories:

  • Elevated body mass index (BMI): A BMI of 30 or above, or a BMI of 27 or above with at least one weight-related comorbidity such as type 2 diabetes, hypertension, dyslipidemia, or obstructive sleep apnea.
  • Weight-related health conditions: Prediabetes, metabolic syndrome, joint pain limiting activity, fatty liver disease, and other conditions that may improve with weight reduction.
  • History of unsuccessful attempts: Prior efforts with diet and exercise alone that did not produce lasting results, particularly when medical factors may not have been addressed.
  • Need for prescription treatment: Clinical profiles in which provider-guided medical therapies may be indicated and safe to use under supervision.
  • Preparation for or recovery from bariatric surgery: Some patients seek medical support before surgical evaluation or during post-operative maintenance.

BMI is a screening tool, not a perfect measure of health. Athletes with higher muscle mass and individuals who carry weight differently may require clinical judgment beyond a BMI calculation alone. That is one reason a provider evaluation matters.

When Medical Weight Loss May Not Be the First Step

Medical supervision is not always necessary or appropriate. You may not need a medical program if:

  • You have a modest amount of weight to lose and no significant weight-related health conditions
  • You respond well to nutrition counseling or structured lifestyle changes without medical intervention
  • You are currently pregnant or breastfeeding
  • Active untreated eating disorder behaviors require specialized psychiatric or nutritional care first
  • Certain unstable medical conditions need to be addressed before weight loss treatment begins

In some cases, your provider may recommend addressing a specific health concern before starting a weight loss program, or coordinating care with your primary care physician or specialist.

What a Candidacy Evaluation Includes

Determining candidacy is not a yes-or-no decision made from a form or online quiz. A proper evaluation typically includes:

  • Review of your complete medical and surgical history
  • Current medication and supplement list
  • Family history of obesity, diabetes, and cardiovascular disease
  • Discussion of prior weight loss attempts and outcomes
  • Physical assessment and vital signs
  • Laboratory testing when clinically indicated
  • Conversation about your goals, timeline expectations, and readiness for change

This process helps your provider determine not only whether you are a candidate for medical weight loss, but which components of a supervised program—nutrition guidance, activity planning, medications, clinic therapies—may be appropriate for you specifically.

Candidacy Evaluations in California

Patients throughout California can schedule an in-person candidacy evaluation at one of our clinic locations. Your licensed provider reviews your health history, discusses your goals, and determines whether physician-supervised weight management is appropriate. Some clinical situations may warrant additional lab work during your visit or at a local facility.

Special Populations and Considerations

Certain groups require additional clinical attention when evaluating candidacy:

  • Patients with diabetes: Weight loss can improve blood sugar control, but medication adjustments may be needed to prevent hypoglycemia.
  • Patients on multiple medications: Drug interactions must be reviewed before starting weight loss treatments.
  • Older adults: Sarcopenia (age-related muscle loss) and frailty risk may influence how aggressively weight is reduced.
  • Patients with cardiovascular disease: Treatment selection and monitoring intensity may differ from standard protocols.

These are not automatic exclusions from medical weight loss—they are factors that shape how your plan is designed and monitored.

Your Next Step

If you recognize yourself in the criteria above and wonder whether medical support could make a difference, the most useful next step is a consultation with a licensed provider. Candidacy is ultimately a clinical decision made in partnership with you, based on your individual health data—not a marketing claim or online assessment.

Individual results vary. A candidacy evaluation does not commit you to treatment; it provides the information you need to make an informed decision about your care.
